Retina Detachment in Singapore: Symptoms, Treatment and Recovery

Retina detachment is a sight threatening eye emergency that requires prompt assessment and, in most cases, urgent treatment. 

A retina detachment occurs when the retina separates from the underlying tissues that support it. Without timely treatment, permanent visual loss can occur.

Dr Paul Zhao is a fellowship-trained vitreoretinal surgeon and retina specialist who specialises in retina detachment surgery and other vitreoretinal conditions. He assesses patients with suspected retinal detachment and performs major surgical procedures used to repair detached retinas.

What are the warning signs of a retina detachment?

  • SUDDEN SHOWER OF FLOATERS

  • FLASHES OF LIGHT

  • A DARK CURTAIN OR SHADOW ACROSS YOUR VISION

  • LOSS OF PART OF YOUR VISUAL FIELD

  • SUDDEN DETERIORATION OF VISION

  • DISTORTION OR BLURRING OF VISION

If you experience a sudden increase in floaters or flashes particularly if accompanied by a curtain, shadow or loss of part of your vision, you should seek urgent assessment by a retina specialist.

What are the types of retina detachment?

Rhegmatogenous retina detachment

The most common type. A retina tear or break allows fluid to pass underneath the retina, causing it to detach.

Traction retina detachment

The retina is pulled away from the underlying tissue by scar tissue. This can occur in advanced diabetic retina disease

Exudative retina detachment

Fluid accumulates underneath the retina without a retina tear. It can occur because of inflammatory, vascular, tumour related or other conditions.

How is a retina detachment diagnosed?

A retina specialist will perform a dilated examination of the retina to determine:

  • whether the retina is detached

  • where the retina tear or break is located

  • how extensive the detachment is

  • whether the macula is detached

  • whether there are additional retina tears; whether there is associated vitreous haemorrhage or scar tissue.

Additional imaging such as wide-field retinal photography, OCT or B-scan ultrasound may be used when appropriate.

Treatment Options

Which retina detachment surgery is best?
 

There is no single operation that is best for every retina detachment. The choice depends on the location, number of tears, the extent of detachment, whether the macula is involved, the duration of detachment and presence of scar tissue and the other characteristics of the eye.

Vitrectomy For Retina Detachment Repair

This involves removing the vitreous gel in the eye and flattening the retina by injecting a gas bubble. The retina tear is treated with laser. The patient needs to posture face down after surgery.

Scleral Buckling Surgery

A silicone band is sutured to the wall of the eye externally to create an indent that supports the retina break and in turn allows the retina detachment to settle.

Pneumatic Retinopexy

A gas bubble is injected into the eye to seal the retina tear and flatten the retina. Cryopexy or laser is then applied to seal the tear.

What should I expect after retina detachment surgery?

  • Temporary blurred vision is common

  • A gas bubble may be placed inside the eye

  • Positioning instructions depend on the procedure and location of the retinal break

  • Vision may improve gradually over weeks or months

  • Air travel is unsafe if there is an air bubble in the eye

  • Follow-up examinations are important
